Fees

Payment is expected to be made at the time of service prior to beginning session unless other arrangements are made.  Dr. Sarazin accepts cash, checks, and credit cards, and has a limited number of sliding scale openings that cannot be guaranteed to be available to new patients.  All sessions are 50 minutes unless you make other scheduling arrangements with Dr. Sarazin.  Cancellation of sessions with less than 24 hours notice or failing to show for a scheduled session will be billed at the regular session rate.  All patients are strongly encouraged to sign up for appointment reminders via the client portal to help themselves avoid late cancellation charges.

Insurance

Dr. Sarazin provides services to insurance patients as an out-of-network provider via the process of providing patients with a superbill so that they may receive reimbursement directly from their own insurance provider.  Please be aware that the responsibility belongs to the patient to understand how their benefits do or do not apply to my services prior to starting therapy, as billing will begin with services provided.

For plans that permit out-of-network providers (e.g., most PPO plans, some BCBS plans, Anthem Blue Cross, SISC, etc.).  Please contact your insurance company to confirm your out-of-network benefits if you plan to submit a superbill.  It is particularly important to understand how your deductible works, and to understand that for percentage based reimbursement your insurance company is often referring to a percentage of their internally established “usual and customary” rate and NOT NECESSARILY a percentage of the actual fee.

This fee structure allows patients to be in complete control of their financial relationship with psychological services.  Only the patient and insurance provider have full access to all of the information about the patients specific plan benefits, as well as any other strategies the patient may use to cover relevant costs (e.g., funding an HSA or flexible benefits plan).  When billing insurance companies directly in the past, Dr. Sarazin has found that as a provider he has not had enough access to information regarding individuals’ benefits to be able to consistently give accurate information to patients about what they can expect to pay.

When expectations are not met, or communication is inaccurate, this causes strain on the therapy relationship and can be very unhelpful to the clinical process.  Other problems have come up when insurance companies have made unilateral changes to reimbursement without communication to the patient or provider, again inserting themselves into the therapy relationship in intrusive and unhelpful ways.  The purpose of the therapeutic encounter is to focus on and explore the relationship and the clinical work, which can be difficult and complicated enough without having to spend precious time problem-solving an issue with third party that exists only outside of the consulting room.

Court

As a patient, it is expected that you inform Dr. Sarazin as soon as you are aware that there is a legal issue in your life that may reasonably involve a question in court about psychological functioning.  The most common examples of this are divorce and child custody concerns.

A court retainer fee is billed at $1,000 per half-day, in half-day increments, with payment due two weeks prior to the court date. The retainer will be fully refunded if cancellation of the appearance is provided with at least three full days notice, and refunded at 50% if cancellation is provided at least 24 hours ahead of time.  Weekends are not included days for the purpose of cancelling a court appearance, only business days will be considered (this includes the 24-hour window for 50% refund).  The retainer is the financial responsibility of the party requesting the court appearance.

Dr. Sarazin is available Monday through Friday for regular appointments during the following hours.
